This article featuring India constitutes one of five articles in a collection of essays on local capacity-building in research ethics by graduates from the University of Toronto’s Joint Centre for Bioethics MHSc in Bioethics, International Stream program funded by the Fogarty International Center for Advanced Study in the Health Sciences. Research ethics is a growing area of work and interest in India. Ethics review remains the weakest component in the mechanism of good clinical practice, and there is a severe dearth (...) of professionals trained in ethics who can provide leadership. Although the Indian Good Clinical Practice Guidelines, the Indian Medical Council Act, and the Drugs and Cosmetics Act require that the Indian Council of Medical Research’s ethical guidelines be followed as a mandatory requirement for physicians who conduct research, there is a pervasive lack of awareness of basic requirements guiding the ethical conduct of research. Trafficking and persistence of fetal microchimeric cells (fMCs) and circulating extracellular vesicles (EVs) have been observed in animals and humans, but their consequences in the maternal body and their mechanistic contributions to maternal physiology and pathophysiology are not yet fully defined. Fetal cells and EVs may help remodel maternal organs after pregnancy‐associated changes, but the cell types and EV cargos reaching the mother in preterm pregnancies after exposure to various risk factors can be distinct from term pregnancies. As preterm delivery‐associated (...) maternal complications are rising, revisiting this topic and formulating scientific questions for future research to reduce the risk of maternal morbidities are timely. Epidemiological studies report maternal cardiovascular risk as one of the major complications after preterm delivery. This paper suggests a potential link between fMCs and circulating EVs and adverse maternal cardiovascular outcomes post‐pregnancies, the underlying mechanisms, consequences, and methods for and how this link might be assessed. Similarity profiled association mining from time stamped transaction databases is an important topic of research relatively less addressed in the field of temporal data mining. Mining temporal patterns from these time series databases requires choosing and applying similarity measure for similarity computations and subsequently pruning temporal patterns. This research proposes a novel z-space based interest measure named as Krishna Sudarsana for time-stamped transaction databases by extending interest measure Srihass proposed in previous research. Krishna Sudarsana is designed by using the product (...) based fuzzy Gaussian membership function and performs similarity computations in z-space to determine the similarity degree between any two temporal patterns. The interest measure is designed by considering z-values between z = 0 and z = 3.09. Applying the Krishna Sudarsana requires moving the threshold value given by user to a different transformation space which is a defined as a function of standard deviation. In addition to proposing interest measure, new expressions for standard deviation and equivalent z-space threshold are derived for similarity computations. For experimental evaluation, we considered Naïve, Sequential and Spamine algorithms that applies Euclidean distance function and compared performance of these three approaches to Z-Spamine algorithm that uses Krishna Sudarsana by choosing various test cases. Experiment results proved the performance of the proposed approach is better to Sequential approach that uses snapshot database scan strategy and Spamine approach that uses lattice based database scan strategy. For over a century, Haeckel's Gastraea theory remained a dominant theory to explain the origin of multicellular animals. According to this theory, the animal ancestor was a blastula‐like colony of uniform cells that gradually evolved cell differentiation. Today, however, genes that typically control metazoan development, cell differentiation, cell‐to‐cell adhesion, and cell‐to‐matrix adhesion are found in various unicellular relatives of the Metazoa, which suggests the origin of the genetic programs of cell differentiation and adhesion in the root of the Opisthokonta. Multicellular (...) stages occurring in the complex life cycles of opisthokont protists (mesomycetozoeans and choanoflagellates) never resemble a blastula. Here, we discuss a more realistic scenario of transition to multicellularity through integration of pre‐existing transient cell types into the body of an early metazoon, which possessed a complex life cycle with a differentiated sedentary filter‐feeding trophic stage and a non‐feeding blastula‐like larva, the synzoospore. Felicitas Goodman (1990) observed that naive participants experienced unique trance states, characterized by specific visionary content, when they assumed particular postures and listened to monotonous rattling. Students (n = 284), enrolled in various sections of the course Introduction to Psychology, experienced one of four conditions with their eyes closed: Sitting Quietly with and without Drumming, Standing (Feather Serpent) Posture plus Drumming with and without Suggested Experiences. Participants completed the Phenomenology of Consciousness Inventory (Pekala 1982, 1991c) and wrote narratives following their (...) respective stimulus conditions. Participants reported less imagery, joy, and absorption, but more internal dialogue, self‐awareness, and muscular tension in the posture, compared to the sitting quietly, conditions. Goodman's results of specific posture (content‐related) effects with naive participants were not replicated. (shrink)

This commentary reflects on the varieties of high hypnotizable subjects suggested in the works by Barber, Barrett, Pekala and colleagues, and Terhune and Cardeña . These different studies point to the existence of different types of low, medium, and high hypnotizable subjects. However, types of high hypnotizables have received the most attention. Two main concerns are raised in this commentary: drawing parallels between the suggested typologies is not without problems given methodological differences among different studies, and the low base rates (...) of these special types is likely not to appeal to a typical clinician, already resistant to testing for hypnotizability, to conduct initial assessments so as to tailor suggestion to fit specific typologies. Making use of the SNAP and SPAN ontologies, we show how three reference domain ontologies can be integrated at a higher level, through what we shall call the OBR framework (for: Ontology of Biomedical Reality). The most predominant kind of disease that is normal among ladies is breast cancer. It is one of the significant reasons among ladies, regardless of huge endeavors to stay away from it through screening developers. An automatic detection system for disease helps doctors to identify and provide accurate results, thereby minimizing the death rate. Computer-aided diagnosis has minimum intervention of humans and produces more accurate results than humans. It will be a difficult and long task that depends on the expertise (...) of pathologists. Deep learning methods proved to give better outcomes when correlated with ML and extricate the best highlights of the images. The main objective of this paper is to propose a deep learning technique in combination with a convolution neural network and long short-term memory with a random forest algorithm to diagnose breast cancer. Here, CNN is used for feature extraction, and LSTM is used for extracted feature detection. The experimental results show that the proposed system accomplishes 100% of accuracy, a sensitivity of 99%, recall of 99%, and an F1-score of 98% compared to other traditional models. As the system achieved correct results, it can help doctors to investigate breast cancer easily. (shrink)

The notion of epistemic injustice has become an important topic of inquiry in recent times. It refers to the injustice committed to a person when her claim to knowledge is not given due consideration. This article argues that there are two major sources of epistemic injustice: One is the dominating tendencies present in us, and the other is susceptibility to cognitive biases and distortions. When societies become more complex, injustice increases and one can see countless instances of epistemic injustice in (...) everyday life. To reduce epistemic injustice, one has to tackle both sources. Increasing cooperative behaviour is the key in this regard which, in turn, may require revisiting the way the self is automatically understood. (shrink)

Background: Meditation is gaining recognition as a tool to impact health and well-being. Samyama is an 8-day intensive residential meditation experience conducted by Isha Foundation requiring several months of extensive preparation and vegan diet. The health effects of Samyama have not been previously studied. The objective was to assess physical and emotional well-being before and after Samyama participation by evaluating psychological surveys and objective health biomarkers.Methods: This was an observational study of 632 adults before and after the Isha Samyama retreat. (...) All participants were invited to complete surveys. Controls included household significant others. Surveys were completed at baseline, just before Samyama, immediately after Samyama, and 3 months later to assess anxiety, depression, mindfulness, joy, vitality, and resilience through validated psychometric scales. Voluntary blood sampling for biomarker analysis was done to assess hemoglobin, HbA1c, lipid profile, and C-reactive protein. Primary outcomes were changes in psychometric scores, body weight, and blood biomarkers.Results: Depression and anxiety scores decreased from T1 to T3, with the effect most pronounced in participants with baseline depression or anxiety. Scores at T4 remained below baseline for those with pre-existing depression or anxiety. Vitality, resilience, joy, and mindfulness increased from T1 to T3. Body weight decreased by 3% from T1 to T3. Triglycerides were lower from T2 to T3. Participants had lower HbA1c and HDL at T2, and lower CRP at all timepoints compared with controls.Conclusions: Participation in the Isha Samyama program led to multiple benefits. The 2-month preparation reduced anxiety, and participants maintained lower anxiety levels at 3 months post-retreat. Physical health improved over the course of the program as evidenced by weight loss and improved HbA1C and lipid profile. Practices associated with the Samyama preparation phase and the retreat may serve as an effective way to improve physical and mental health. Future studies may examine their use as an alternative therapy in patients with depression and/or anxiety.Clinical Trial Registration:www.ClinicalTrials.gov, Identifier: 1801728792. Speaker recognition under mismatched conditions is a challenging task. Speech signal is nonlinear and nonstationary, and therefore, difficult to analyze under realistic conditions. Also, in real conditions, the nature of the noise present in speech data is not known a priori. In such cases, the performance of speaker identification or speaker verification degrades considerably under realistic conditions. Any SR system uses a voice activity detector as the front-end subsystem of the whole system. The performance of most VADs deteriorates at the (...) front end of the SR task or system under degraded conditions or in realistic conditions where noise plays a major role. Recently, speech data analysis and processing using Norden E. Huang’s empirical mode decomposition combined with Hilbert transform, commonly referred to as Hilbert–Huang transform, has become an emerging trend. EMD is an a posteriori, adaptive, data analysis tool used in time domain that is widely accepted by the research community. Recently, speech data analysis and speech data processing for speech recognition and SR tasks using EMD have been increasing. EMD-based VAD has become an important adaptive subsystem of the SR system that mostly mitigates the effect of mismatch between the training and the testing phase. Recently, we have developed a VAD algorithm using a zero-frequency filter-assisted peaking resonator and EMD. In this article, the efficacy of an EMD-based VAD algorithm is studied at the front end of a text-independent language-independent SI task for the speaker’s data collected in three languages at five different places, such as home, street, laboratory, college campus, and restaurant, under realistic conditions using EDIROL-R09 HR, a 24-bit wav/mp3 recorder. The performance of this proposed SI task is compared against the traditional energy-based VAD in terms of percentage identification rate. In both cases, widely accepted Mel frequency cepstral coefficients are computed by employing frame processing from the extracted voiced speech regions using the respective VAD techniques from the realistic speech utterances, and are used as a feature vector for speaker modeling using popular Gaussian mixture models. The experimental results showed that the proposed SI task with the VAD algorithm using ZFFPR and EMD at its front end performs better than the SI task with short-term energy-based VAD when used at its front end, and is somewhat encouraging. Purpose The purpose of this study is to examine how the adoption of blockchain technology can improve organizational sustainability and what are the contributions of morality, ethics and governance in this scenario. Design/methodology/approach This study has used different literature and theories to build a successful theoretical model and then validated it using the partial least squares structural equation modeling approach. Various statistical modeling analyses have been performed to test the robustness of the proposed model, which is found to be effective (...) and unique as it has a high explanatory power. Findings This study has found that adoption of blockchain technology can improve the sustainability performance of organizations. It has also highlighted that organizational sustainability will be further enhanced if the underlying algorithms of blockchain technology are transparent to make the applications moral and ethical. This study has further demonstrated that appropriate governance of blockchain technology can support the organizations to enhance their sustainability performance. Research limitations/implications This study demonstrates the importance of an organization’s governance over blockchain technology and its impact on sustainability. The study provides valuable input to practitioners and policy makers regarding blockchain technology and its governance models. Also, the study provides valuable recommendations to leaders and managers of organizations on how to successfully adopt blockchain technology. Originality/value This study presents a unique theoretical model to explain the influence of the adoption of blockchain technology and an organization’s sustainability. BackgroundHealthcare ethics is neglected in clinical practice in LMICs such as Nepal. The main objective of this study was to assess the current status of knowledge, attitude and practice of healthcare ethics among resident doctors and ward nurses in a tertiary teaching hospital in Nepal.MethodsThis was a cross sectional study conducted among resident doctors and ward nurses in the largest tertiary care teaching hospital of Nepal during January- February 2016 with a self-administered questionnaire. A Cramer’s V value was assessed to (...) ascertain the strength of the differences in the variables between doctors and nurses. Association of variables were determined by Chi square and statistical significance was considered if p value was less than 0.05.ResultsOur study demonstrated that a significant proportion of the doctors and nurses were unaware of major documents of healthcare ethics: Hippocratic Oath, Nuremberg code and Helsinki Declaration. A high percentage of respondents said that their major source of information on healthcare ethics were lectures, books, and journals. Attitude of doctors and nurses were significantly different in 9 out of 22 questions pertaining to different aspects of healthcare ethics. More nurses had agreement than doctors on the tested statements pertaining to different aspects of healthcare ethics except for need of integration of medical ethics in ungraduate curricula,paternalistic attitude of doctor was disagreed more by doctors. Notably, only few doctors stood in support of physician-assisted dying.ConclusionsSignificant proportion of doctors and nurses were unaware of three major documents on healthcare ethics which are the core principles in clinical practice. Provided that a high percentage of respondents had motivation for learning medical ethics and asked for inclusion of medical ethics in the curriculum, it is imperative to avail information on medical ethics through subscription of journals and books on ethics in medical libraries in addition to lectures and training at workplace on medical ethics which can significantly improve the current paucity of knowledge on medical ethics. India has one of the most heterogeneous societies in the world. It is a multi-cultural, multi-linguistic, multi-ethnic and multi-religious country. Constitutionally, it is also a sovereign socialist secular democratic republic. But in recent times, Hindu nationalism or Hindutva has been dominant in shaping Indian politics. Hindutva, a shorthand of Hindu nationalism, is actually a politico-ideological device that appears to be disassociated from the spiritual roots of Hinduism and, to many, it is very much alike to the rise of political Islam. (...) Although Hinduism is native to the Indian subcontinent, Hindutva as a political ideology is comparatively a recent phenomenon which creates a growing concern to the land since 1920s. India experienced the rise of Hindutva as a concrete political ideology in 1923 introduced by V. D. Savarkar as it starts to achieve its popularity since 1980s. Obviously it has many reasons. Hindutva is not a monolithic concept as it is generally perceived. Rather, its text, subtext and context had changed throughout the 20th century depending on the period and leadership. This seminar report discusses the low-voltage current-mode analog circuits and their various aspects. The need of high speed, high performance, low power circuits because of the advent of the portable electronic and mobile communication systems and difficulties faced in achieving that in today’s scenario are presented. Current mode circuits are the best suited candidates for the above. Their advantages are discussed here and a comparison with the conventional voltage mode circuits has been presented. The principle and the implementation of the (...) most common current mode circuits i.e. the current conveyors, has been described. The basic device level techniques also play important role in the design of smarter and efficient circuits. Some of those techniques have also been discussed here. The present work is an attempt to critically analyse different philosophical concepts and theories associated with Husserlian phenomenology Western philosophy has witnessed series of revolutions beginning from Socratic-Platonic tradition to Cartesian and Kantian revolution. BackgroundThe potential contribution of community engagement to addressing ethical challenges for international biomedical research is well described, but there is relatively little documented experience of community engagement to inform its development in practice. This paper draws on experiences around community engagement and informed consent during a genetic cohort study in Kenya to contribute to understanding the strengths and challenges of community engagement in supporting ethical research practice, focusing on issues of communication, the role of field workers in 'doing ethics' on (...) the ground and the challenges of community consultation.MethodsThe findings are based on action research methods, including analysis of community engagement documentation and the observations of the authors closely involved in their development and implementation. Qualitative and quantitative content analysis has been used for documentation of staff meetings and trainings, a meeting with 24 community leaders, and 40 large public and 70 small community group meetings. Meeting minutes from a purposive sample of six community representative groups have been analysed using a thematic framework approach.ResultsField workers described challenges around misunderstandings about research, perceived pressure for recruitment and challenges in explaining the study. During consultation, leaders expressed support for the study and screening for sickle cell disease. In community meetings, there was a common interpretation of research as medical care. Concerns centred on unfamiliar procedures. After explanations of study procedures to leaders and community members, few questions were asked about export of samples or the archiving of samples for future research.ConclusionsCommunity engagement enabled researchers to take account of staff and community opinions and issues during the study and adapt messages and methods to address emerging ethical challenges. Field workers conducting informed consent faced complex issues and their understanding, attitudes and communication skills were key influences on ethical practice. Community consultation was a challenging concept to put into practice, illustrating the complexity of assessing information needs and levels of deliberation that are appropriate to a given study. Following up on a 1989 paper on the subject, this essay revisits the question of ethical expertise in the court room. Informed by recent developments in the use of ethics experts, the authors argue 1) that the adversarial nature of court proceedings challenges the integrity of the ethicist's pedagogical role; 2) that the use of ethics experts as normative authorities remains dubious; 3) that clarification of the State's interest in “protecting the ethical integrity of the medical profession” is urgently required; (...) and 4) that the expertise of the ethicist may be more appropriately used in advising the legislature than in influencing the court. The study of the autonomic nervous system (ANS) function has shown to provide useful indicators for risk stratification and early detection on a variety of cardiovascular pathologies. However, data gathered during different tests of the ANS are difficult to analyse, mainly due to the complex mechanisms involved in the autonomic regulation of the cardiovascular system (CVS). Although model-based analysis of ANS data has been already proposed as a way to cope with this complexity, only a few models coupling the main (...) elements involved have been presented in the literature. In this paper, a new model of the CVS, representing the ventricles, the circulatory system and the regulation of the CVS activity by the ANS, is presented. The models of the vascular system and the ventricular activity have been developed using the Bond Graph formalism, as it proposes a unified representation for all energetic domains, facilitating the integration of mechanic and hydraulic phenomena. In order to take into account the electro-mechanical behaviour of both ventricles, an electrophysiologic model of the cardiac action potential, represented by a set of ordinary differential equations, has been integrated. The short-term ANS regulation of heart rate, cardiac contractility and peripheral vasoconstriction is represented by means of continuous transfer functions. These models, represented in different continuous formalisms, are coupled by using a multi-formalism simulation library. Results are presented for two different autonomic tests, namely the Tilt Test and the Valsalva Manoeuvre, by comparing real and simulated signals. (shrink)

Enabling constraints are bottom up causes which create the possibility of the existence of a system. Disabling constraints reduce the degrees of freedom and narrow the choices of the system which are structural, functional, meaningful relations that assign executive roles to the component parts. In this paper, we discuss causality as enabling and disabling constraints in order to critique the absurdity of transitivity in causal relations. If downward causation is viewed as causation by constraints, we argue that it will not (...) face the absurdity of self-causation which is incurred because of the transitivity principle. In the process of self-organization, organizationally new structures are created from the lower level subcomponents of the system. Such self-organizing systems can be broadly considered as sui-generis and transcend the transitivity principle by causally influencing its subcomponents. These higher level systemic constraints that causally influence the components in a self-organizing system without violating any physical laws via causal constraints are termed as Downward Causation.
